New Employee Information

We are looking forward to your employment here at State Park Marina, and ask that the following documents be turned-in to your supervisor before your first day of work:

  1. Federal W-4

  2. I-9 and Supporting Documents a. US Passport, Valid Driver's License, School Photo ID, or Voter's Registration Card, AND b. Original Birth Certificate or Social Security Card

  3. Employee Information Form

  4. Read Crew Manual and sign Acknowledgement

  5. Job Description, signed on page 2 for Store Crew and page 4 for Dock Crew

  6. Copy of Missouri State Water Patrol Boating Safety Education Card, if born after 1/1/1984. You can start the online course HERE

Outside Service Provider Information

Beginning April 14, 2014, State Park Marina is going to require 
more information from all outside service providers.  
The new requirements are below, as well as a copy of the full document with signature requirements.
Please be mindful of these requirements when choosing service providers.

OUTSIDE CONTRACTOR REQUIREMENTS
These rules apply to ALL Outside Contractors Working at State Park Marina, including Service & Repair Companies, Vendors, and Boat Dealers & Brokers.  Companies and individuals who are not employees by this Marina are welcome to work on boats at our Marina as long as they understand and comply with all STATE PARK MARINA rules.  Outside professionals must follow the same rules we work with for security, safety and environmental protection reasons.
To Keep the Marina operating in a safe, clean, and professional manner, outside professionals MUST comply to the provisions of our Outside Contractor Safety Agreement and providing the marina office with copies of:
1. Current occupational license and city/state sales tax registration numbers.
2. Current certificate of public liability insurance (minimum $1,000,000) with companies reasonably acceptable to this Marina.  The Marina office shall be furnished with copies of such policies, which shall contain a provision that cancellation or reduction in coverage thereof shall not be effective as to this Marina except upon 30 days advance written notice to the Marina office.
3. Current Workman’s Compensation Insurance Certificate.
4.      Signed copy of the Outside Contractors Safety Agreement and Indemnity and Assumption of Risk; signature should be that of an authorized member of management for your company.
5.      Copies of all MSDS Sheets for all chemicals being used by the Contractor while in the Marina and kept current as new products are used.
6.      A signed copy of the Outside Contractor Safety Agreement must be on file in the Marina Office before starting work in this Marina.
7.      Prior to starting any work, the outside professional or agent must report to the Marina’s office each day to confirm current status of the required documents.
8.      Failure to report prior work or to not have all documents current and on file, will result in suspension of access to the Marina.

Fuel Club 2013

State Park Marina would like to sincerely thank you for your business this past boating season. The Fuel Club was once again a great success and we owe that to you, our great customers!

All customers purchasing at least $50 in fuel should receive two forms from State Park Marina by the end of January. Additional forms required by the Missouri Department of Revenue to receive your $0.17 per gallon road tax refund, are Forms 4923, 4924, and 4925. These forms described below, can be filled out online by clicking each form number.

FORM 4923 – The Fuel Refund Claim – this form includes specific invoice information and needs to be submitted for each claim. This form may be filed as often as you wish, but must be filed within one year from the date of purchase, or by April 15th following the year of purchase, whichever is later.

FORM 4924 - The Fuel Tax Refund Application – asks for basic information such as address, phone number, etc. The form will need to be submitted every two years with your claim.

FORM 4925 - Marine Fuel Purchases Schedule A– This form is used to apply for a refund of tax for marine use. Please attach your State Park fuel purchases printout to this form.

Please mail all forms and paperwork including:
(1) Form 4923, Form 4924, and Form 4925
(2) SPM Fuel Sales Printout
(2) Letter to MO DOR from SPM

NEW Recycling Stations

New this year are recycling stations at State Park Marina. In an effort to reduce waste, the marina will be offering five green comingled recycling receptacles around the marina.

These stations WILL accept the following:
  • Opened mail & greeting cards
  • Paper egg cartons
  • Paperboard boxes
  • File Folders
  • Shredded paper
  • Magazines, brochures & catalogs
  • White or pastel office paper
  • Newspapers & inserts
  • Phone books
  • Cans (do not crush)
  • #1-7 Plastic tubs & screw-top jars, tubs, and plastic bottles (remove lids)
  • Clean, balled aluminum foil and pie pans
  • Pizza boxes
  • Loose metal jar lids & steel bottle caps
  • Flattened corrugated cardboard & paper bags
  • Paper milk/juice cartons
  • Paper & Hard-back books
  • Empty aerosol cans (no caps)

There's no need to remove paper clips, stamps, address labels, staples, tape, wire, metal fasteners, rubber bands, spiral bindings, or plastic tabs.


Please empty and rinse all containers, but do not flatten.

THE DIRTY DOZEN
The following list reflects the worst recycling contaminants that can NOT go into the recycling bin:
  1. NO Plastic Bags: These are the WORST contaminant in the recycling bin and can be recycled at places such as Wal-Mart, but not here at SPM.
  2. NO Materials in Plastic Bags: Don't put your recyclables in plastic bags; please empty contents into bin.
  3. NO Plastic lids or caps: please remove from plastic bottles
  4. NO GLASS OF ANY KIND
  5. NO Liquids
  6. NO Ceramics
  7. NO Diapers or Bio-Hazardous Waste such as needles or syringes
  8. NO Hazardous Waste such as paint, automotive fluids, batteries, or pesticides
  9. NO Scrap Metal of any kind
  10. NO Frozen food boxes: These were designed with a plastic polymer to protect the food against freezer burn and is not recyclable or compostable.
  11. NO light bulbs, plates or vases
  12. NO Tissues, paper towels, or napkins
